At Counterpart Health , we are transforming healthcare and improving patient outcomes through Counterpart Assistant, our innovative primary care technology. By empowering primary care physicians with actionable clinical insights, we help practices identify disease earlier, manage chronic conditions longitudinally, and deliver higher-quality care at a lower cost.

We are seeking a Senior Provider Success Manager to serve as a strategic advisor to primary care organizations in New Jersey using Counterpart Assistant. This individual will own the success of a complex portfolio of provider partnerships and lead the strategies that drive sustained adoption, measurable performance improvement, expansion, and long-term retention.

Beyond managing individual partnerships, this person will identify patterns across the market, develop scalable provider-success strategies, and mentor colleagues on effective engagement and change-management practices. The role requires strong judgment, comfort navigating ambiguity, and the ability to lead complex initiatives without direct authority.

Candidates must reside currently reside in New Jersey and be willing to travel to provider practices up to 85% of the time.
